    How do you start a scene where two people are talking in a car?

    Would it be

    INT. CAR - DAY

    INT./EXT. CAR - DAY

    or

    EXT. A ROAD - IN A CAR - DAY

    or none of these?

    it would be

    INT. CAR - DAY

    and then you start the action/dialogue.

    INT/EXT. CAR - DAY

    implies you start the scene in the car as they're getting out, and they end up out.

    EXT. A ROAD - IN A CAR - DAY

    suggests that the road is inside the car.

        'Bout two hours. You okay?

        She smiles wearily.

        Suddenly, the motor SPUTTERS. They look at each other, concerned.
