I think a lot of people are getting the "twist" confused with the "ending".
SPOILERS
The "ending" is when Leo realizes that this whole time he was simply trying to be a good man, not a monster (even though, in reality, he was). That line he gives at the end is indication that he knows who he is and what he has done, but he must carry on the lie so that he can be lobotomized and not live any more as a monster.
That's what I took it as, anyway. If it had simply ended with the "twist", I would have been disappointed.
